ABSTRACT

Introdução: mapear os estudos que mensuraram o potencial Hidrogeniônico (pH) da região vulvar no ciclo vital da mulher. Métodos: revisão de escopo conforme recomendações do Manual for Evidence Synthesis do Joanna Briggs Institute (JBI), nas bases SCOPUS, Web of Science, Academic Search Premier, PubMed, Bielefeld Academic Search Engine e Google Acadêmico em janeiro de 2022. Foram incluídos estudos publicados em português, espanhol, francês e inglês, sem limite temporal. Resultados: dos 954 documentos recuperados, 13 foram selecionados. Apurou- se que os estudos utilizaram diferentes procedimentos para as medições quanto aos equipamentos, ambiente e preparo das participantes. A média de idade apresentada pela população feminina estudada variou entre 31 e 43 anos. Os locais de medição vulvar foram os grandes e pequenos lábios, dobra interlabial e períneo. O menor valor do pH aferido foi 4,6 e o maior 6,3. Conclusões: constatou-se baixa produção de estudos acerca do pH vulvar e ênfase das pesquisas na população de mulheres adultas. A diversidade de procedimentos e locais de aferição encontrados não permite afirmações seguras sobre uma faixa de valor de pH da superfície da pele vulvar.


Introduction: to map the studies that measured the Potential of Hydrogen (pH) of the vulvar region in women's life cycle. Methods: scoping review according to recommendations from the Joanna Briggs Institute (JBI) Manual for Evidence Synthesis in the SCOPUS, Web of Science, Academic Search Premier, PubMed, Bielefeld Academic Search Engine and Google Scholar databases in January 2022. Studies published in Portuguese, Spanish, French, and English, without time limit, were included. Results: of the 954 documents retrieved, 13 were selected. Different measurement procedures in relation to equipment, environment and preparation of participants were used in the studies. The average age of the female population ranged between 31 and 43 years. The vulvar measurement sites were the labia majora and minora, interlabial sulci and perineum. The lowest pH value measured was 4.6 and the highest was 6.3. Conclusions: there was a low production of studies on vulvar pH and an emphasis of studies on the population of adult women. The diversity of procedures and measurement sites found does not allow for safe statements about a range of pH values on the surface of the vulvar skin.

La edad media de la población femenina osciló entre 31 y 43 años. Los sitios de medición vulvar fueron los labios mayores y menores, el pliegue interlabial y el perineo. El valor de pH más bajo medido fue 4,6 y el más alto fue 6,3. Conclusiones: hubo una baja producción de estudios sobre pH vulvar y un énfasis de estudios en población de mujeres adultas. La diversidad de procedimientos y sitios de medición encontrados no permite realizar afirmaciones seguras sobre un rango de valores de pH en la superficie de la piel vulvar.

ABSTRACT

Fibroma mole, ou pólipo fibroepitelial, é uma lesão de proporções geralmente reduzidas, de cor hiperpigmentada ou igual à da pele, localizando-se frequentemente na face, pescoço, tronco e regiões intertriginosas. É um tumor classificado como benigno e pode acometer tanto homens quanto mulheres em idade reprodutiva e depois da quarta década de vida. Ocorre principalmente em obesos, diabéticos e durante a gestação. Com menor frequência, podem alcançar dimensões que excedem 5 cm. Seu crescimento pode ser lento ou rápido e comumente são assintomáticos, mas podem promover sangramentos por conta de ulcerações decorrentes de traumas repetidos. Apresentamos neste relato um fibroma mole, gigante, de localização vulvar, com 11 cm de comprimento, 11 cm de largura e 5 cm de espessura, pesando 500 g.


Giant soft vulvar fibroma is a fibroepithelial polyp lesion with generally reduced proportions, with a hyperpigmented color or similar to that of the skin, frequently located on the face, neck, trunk and intertriginous regions. It is a tumor classified as benign, can affect both men and women, of reproductive age and after the fourth decade, mainly obese, diabetic and during pregnancy. However, less frequently, they can reach dimensions that exceed 5 cm, may have a slow or accelerated evolution. They are commonly asymptomatic, but bleeding may be present due to ulcerations resulting from repeated trauma. In the current study, we describe a giant soft fibroma with a vulvar location measuring 11 cm in length, 11 cm in width, 5 cm in thickness and weighing 500 grams.

ABSTRACT

Introducción: Los tumores malignos de vulva son tumores raros del tracto genital femenino, que representan menos del 1 % de los tumores malignos del sexo femenino, cerca del 95% de estos son carcinoma escamo celulares, los adenocarcinomas son el tipo histológico menos frecuente, con 36 casos reportados hasta el año 2020. Caso Clínico: Presentamos el caso de una mujer de 36 años con antecedente de histerectomía total por miomatosis, con cuadro clínico de 1 año de evolución caracterizado por una masa indolora en labio mayor izquierdo la cual se somete a biopsia que reporta adenocarcinoma motivo por el cual se somete a vulvectomia radical, en resultado patológico definitivo se confirma el diagnostico de adenocarcinoma de vulva tipo glándula mamaria. Evolución: Se realiza vulvectomía radical + vaciamiento ganglionar inguino-iliaco bilateral. El estu-dio histo-patológico definitivo reveló adenocarcinoma de vulva tipo glándula mamaria pobremente diferenciado G3, Unifocal, con metástasis en 22/25 ganglios derechos y 30/30 izquierdos. Un estu-dio reportó recidiva local extensa, metástasis ganglionares pélvicas, retroperitoneales, mediastinales, axilares, cervicales y pulmonar. Se inició Paclitaxel 137 mg semanal. Se completa quimioterapia hasta 11ra semana posterior a lo cual presentó infección de lecho tumoral y fallece. Conclusión: En el presente caso el curso clínico de adenocarcinoma de vulva tipo glándula mamaria fue agresivo con una supervivencia inferior a 5 meses


Introduction: Malignant tumors of the vulva are rare tumors of the female genital tract, which rep-resent less than 1% of malignant tumors of the female sex, about 95% of these are squamous cell carcinoma, adenocarcinomas are the least frequent histological type, with 36 cases reported until 2020. Clinical Case: We present the case of a 36-year-old woman with a history of total hysterectomy due to myomatosis, with a clinical picture of 1 year of evolution characterized by a painless mass in the left labia majora which undergoes a biopsy that reports adenocarcinoma, which is why undergoes radical vulvectomy, the definitive pathological result confirms the diagnosis of adenocarcinoma of the vulva type mammary gland. Evolution: Radical vulvectomy + bilateral inguino-iliac lymph node dissection was performed. The definitive histopathological study revealed poorly differentiated G3, Unifocal mammary gland adeno-carcinoma of the vulva, with metastases in 22/25 right and 30/30 left lymph nodes. One study reported extensive local recurrence, pelvic, retroperitoneal, mediastinal, axillary, cervical and pulmonary lymph node metastases. Paclitaxel 137 mg weekly was started. Chemotherapy was completed up to the 11th week after which he presented a tumor bed infection and died. Conclusion: In the present case, the clinical course of adenocarcinoma of the vulva type of the mammary gland was aggressive with a survival of less than 5 months.

ABSTRACT

Objetivo: reportar el caso de una paciente con diagnóstico de tejido mamario ectópico en vulva, y realizar una revisión de la literatura acerca del diagnóstico, tratamiento y pronóstico de esta condición en esta localización. Materiales y métodos: paciente de 49 años consulta por masa vulvar dolorosa a un centro privado de nivel medio de complejidad ubicado en Bogotá, Colombia. Se realizó ecografía de la lesión y posteriormente escisión quirúrgica; la histopatología mostró tejido mamario ectópico sin malignidad. Se realizó una búsqueda en las bases de datos PubMed, Embase, Cochrane, LILACS y Scielo, con las palabras clave "Vulva", "Glándulas mamarias", "ectópico", "Vulva", "Breast" y "Ectopic", y se incluyeron reportes y series de caso de mujeres con tejido mamario en vulva confirmado por histopatología. Resultados: se identificaron 184 títulos, de los cuales 94 fueron finalmente incluidos para un total de 126 casos. El 57,9% eran tumores benignos, con un 95% en mujeres menores de 50 años; y el 42,06% eran tumores malignos, el 92 % en mujeres mayores de 50 años. El diagnóstico se realizó por clínica, con imágenes diagnósticas complementarias, marcadores tumorales e inmunohistoquímica en algunos casos. Se realizó escisión local en el 91% de los casos de patología benigna y 43% de patología maligna, siendo el método diagnóstico y terapéutico. Conclusión: el tejido mamario ectópico en vulva debe ser considerado como parte del diagnóstico diferencial de masas a nivel vulvar con diferente pronóstico en mujeres pre y posmenopáusicas. Se requieren más estudios para una mejor caracterización de la patología y definir el tratamiento ideal en términos de recaída y supervivencia.


Objective: To report the case of a patient diagnosed with ectopic mammary tissue in the vulva, and to conduct a literature review of the diagnosis, treatment and prognosis of this condition in that location. Material and methods: A 49-year-old patient who presented with a painful vulvar mass to a private intermediate complexity center in Bogotá, Colombia. The lesion was assessed on ultrasound and then surgically excised; histopathology showed ectopic mammary tissue with absence of malignancy. A search was conducted in the PubMed, Embase, Cochrane, LILACS and Scielo databases using the keywords "Vulva," "Breast" and "Ectopic." Case reports and case series of women with histopathology-confirmed mammary tissue in the vulva were included. Results: Overall, 184 titles were identified and, of these, 94 were ultimately included, for a total of 126 cases, with 57.9% being benign tumors, 95% in women under 50 years of age, and 42.06% being malignant tumors, 92% in women over 50 years of age. Diagnosis was made on the basis of the clinical findings, with ancillary diagnostic imaging, tumor markers and immunohistochemistry in some cases. Local excision was performed in 91% of cases with benign pathology and in 43% of cases with malignant pathology, with the diagnostic method being therapeutic. Conclusion: Ectopic mammary tissue in the vulva must be considered as part of the differential diagnosis of vulvar masses, prognosis being different in pre and postmenopausal women. Further studies are needed to enhance the characterization of this condition and define the ideal course of treatment in terms of relapse and survival.

El debut del lupus eritematoso sistémico (LES) durante el embarazo, intrincado con preeclampsia grave, asociado a edema vulvar masivo gestacional, es raro y de difícil diagnóstico. Se reporta una paciente de 19 años, con 35 semanas de gestación, que debutó con LES durante el tercer trimestre del embarazo, y que consulta por manifestaciones cutáneas, dadas por eritemas en pulpejos de dedos de manos. Se constata preeclampsia severa. Se realiza cesárea de urgencia, e ingresa a la unidad de cuidados intensivos. Instala edema de vulva masivo que incapacita la deambulación.


The onset of systemic lupus erythematosus during pregnancy, complicated with severe preeclampsia, associated with massive gestational vulvar edema, is rare and difficult to diagnose. A 19-year-old patient is reported, with 35 weeks of gestation, who started with systemic lupus erythematosus during her third trimester of pregnancy, and consulted due to cutaneous manifestations caused by erythema on the pads of the fingers. Severe preeclampsia was observed. An emergency cesarean section was performed, and the patient was admitted to the Intensive Care Unit. She developed a massive vulvar edema that disabled ambulation.


O aparecimento de lúpus eritematoso sistêmico durante a gravidez, intrincado com pré-eclâmpsia grave é raro e de difícil diagnóstico. É relatada uma paciente de 19 anos, com 35 semanas de gestação, que estreou com lúpus eritematoso sistêmico no terceiro trimestre de gestação, e que se consultou por manifestações cutâneas causadas por eritema nas pontas dos dedos das mãos. Também existe uma pré-eclâmpsia grave. E realizada cesárea de emergência e ela é internada na Unidade de Terapia Intensiva. Instala um edema vulvar maciço que impede a deambulação.

ABSTRACT

Introducción. La hipertrofia de los labios menores se define como tejido labial que sobresale más allá de los labios mayores, puede afectar de manera uni- o bilateral y resultar en dispareunia, interferencia con los deportes, dificultades con la limpieza, irritación e infecciones crónicas del tracto urinario, incomodidad para el uso de prendas ajustadas, además de trastornos psicológicos. Todos estos motivos llevan a las mujeres a la consulta. La reducción de labios menores es el procedimiento estético genital femenino más común. Este trabajo tiene como objetivo presentar técnicas de baja complejidad para resolver diferentes grados de hipertrofia de labios menores. Materiales y métodos. Estudio retrospectivo, observacional que comprendió 3 pacientes entre el periodo de marzo del 2019 a febrero 2020 que Incluyó a pacientes con hipertrofias leves y moderadas. Resultados. En 2 pacientes se realizó escisión directa y en 1, técnica del desepitelizado más lipotransferencia de labios mayores, sin evidenciar complicaciones mayores. Discusión. Si bien en la literatura se han reportado un gran número de técnicas quirúrgicas, cada una de ellas debe adecuarse al tipo de hipertrofia labial. La técnica del desepitelizado permite conservar un borde natural, con conservación de la coloración y textura original de la paciente, aporte neurovascular, pero no es conveniente utilizarla en pacientes de grados mayores de hipertrofia. La escisión directa proporciona una técnica simple para la escisión del exceso de tejido en pacientes con mayor tamaño de sus labios menores, pero elimina el contorno, la coloración y la textura naturales del borde libre, aun así, las pacientes resultan conformes con su nuevo aspecto. Conclusión. La reconstrucción de los labios menores, utilizando las técnicas de desepitelización y escisión directa, es confiable y da un resultado cosmético y funcional exitoso. Estas técnicas son de baja complejidad, tiempo operatorio breve, técnicamente reproducibles, dando gran conformidad y resultando un método seguro


Introduction. Hypertrophy of the labia minora is defined as lip tissue that protrudes beyond the labia majora, can affect unilaterally or bilaterally and result in dyspareunia, interference with sports, difficulties with cleaning, irritation and chronic infections of the urinal tract, discomfort for wearing tight clothes, in addition to psychological disorders. All these reasons lead women to the consultation. Labia minora reduction is the most common female genital cosmetic procedure. This work aims to present low complexity techniques to resolve different degrees of hypertrophy of the labia minora. Materials and methods: retrospective, observational study that comprised 3 patients between the period of March 2019 to February 2020 that included patients with mild and moderate hypertrophy. Results: Direct excision was performed in 2 patients and in 1, de-epithelialization technique plus lipotransference of the labia majora. Without showing major complications. Discussion: Although a large number of surgical techniques have been reported in the literature, each of them must be adapted to the type of lip hypertrophy. The de-epithelialization technique allows a natural border to be preserved, with preservation of the patient's original color and texture, neurovascular supply, but it is not convenient to use it in patients with higher degrees of hypertrophy. Direct excision provides a simple technique for excision of excess tissue in patients with larger labia minora, but removes the natural contour, coloration and texture of the free edge, yet patients are still satisfied with their new appearance. Conclusion. The reconstruction of the labia minora, using de-epithelialization and direct excision techniques is reliable and gives a successful cosmetic and functional result. These are low complexity techniques, short operating time, technically reproducible, giving great conformity and resulting in a safe method

Abstract Objective The present study aimed to explore the opinion and ethical consideration of vulvovaginal aesthetics procedures (VVAPs) among health professionals and medical students in Saudi Arabia. Methods This is a cross-sectional study performed between January 2020 and April 2020. Data was collected through electronic media, WhatsApp, and emails. The results were analyzed by applying the Students t-test, and correlations were considered significant if they presented a p-value<0.05. Results There is significant demand to educate doctors, health professionals, medical students, and gynecologists for the VVAPs to have a solid foundation, justified indications, and knowledge about various aesthetic options. Although female doctors, medical students, young doctors, and gynecologists have more knowledge about VVAPs, all health professionals ought to be aware of recent trends in vulvovaginal aesthetics (VVA). The present analysis determined that VVA should be under the domain of gynecologists, rather than under that of plastic surgeons, general surgeons, and cosmetologists. Themajority of the participants considered that vaginal rejuvenation, "G-spot" augmentation, clitoral surgery, and hymenoplasty are not justifiable on medical grounds. Conclusion The decision to opt for different techniques for vaginal tightening and revitalization should be taken very carefully, utilizing the shared decision-making approach. Ethical aspects and moral considerations are important key factors before embarking in the VVAPs purely for cosmetic reasons. Further research is required to determine the sexual, psychological, and body image outcomes for women who underwent elective VVAPs. Moreover, medical educators must consider VVAPs as part of the undergraduate and postgraduate medical curriculum.

ABSTRACT

Only 14 cases of extraskeletal myxoid chondrosarcoma (EMC) of the vulva have been documented in the literature. We report a case of a 63-year-old woman with EMC of the vulva confirmed by both EWSR1 and NR4A3 fluorescence in situ hybridization, the latter of which is a more specific probe for this entity. The unusual location of this tumor of prominent myxoid morphology gave rise to a wide differential diagnosis, which necessitated thorough histologic evaluation and confirmatory ancillary testing in the form of immunohistochemistry and cytogenetic studies. This article aims to review extraskeletal myxoid chondrosarcoma of the vulva and various diagnostic clues to help differentiate it from its histologic mimics. This is the fifth case of vulvar EMC in the literature with confirmation of a NR4A3 gene rearrangement.

ABSTRACT

Aims: Lipschütz ulcer (LU), also known as acute vulvar ulcer, is a rare cause of vulvar ulcerations of nonvenereal origin. Our aim is to alert about this manifestation of the disease and to prevent unnecessary treatment. Case description: we present a 15 years old female, without relevant family and past history, admitted in the emergency room with a painful vulvar ulcer, preceded by five days of fever and sore throat. On physical examination, she had enlarged, and erythematous tonsils and bilateral anterior cervical lymphadenopathy and the genital examination revealed vulvar oedema and a deep ulcer with necrotic plaques in labium minus. The exclusion of transmitted sexual disease led to a diagnosis of Lipschütz ulcer. She started symptomatic treatment, oral antibiotic and corticoid therapy. She was discharged from the hospital after 6 days of admission and returned to a consult one month later when it was observed an almost complete resolution of the lesions. No recurrences occurred until 3 months. Conclusion: LU is a misdiagnosed pathology, probably because doctors, in general, are not familiarized with that, and since the diagnosis is made by exclusion. Infectious, such as Epstein-Barr Virus infections, are proposed etiologies.


Objetivo: a úlcera de Lipschütz, ou úlcera vulvar aguda, é uma causa rara de ulceração de origem não venérea. O nosso objetivo é alertar para essa manifestação da doença e prevenir tratamentos desnecessários. Descrição: adolescente, sexo feminino, 15 anos, sem história pessoal ou antecedentes familiares de relevo. Recorreu ao Serviço de Urgência por febre e odinofagia, com cinco dias de evolução, associada a uma úlcera vulvar dolorosa, que surgiu no próprio dia. O exame clínico revelou amígdalas hipertrofiadas e eritematosas, linfadenopatia cervical anterior, e o exame ginecológico apresentou edema da vulva e uma úlcera profunda, com placas necróticas nos lábios menores. A exclusão de uma doença sexualmente transmissível levou ao diagnóstico de úlcera de Lipschütz. Ficou internada no Serviço de Pediatria e iniciou tratamento sintomático, antibioterapia por sobreinfeção da úlcera e corticoterapia tópica. Teve alta após seis dias de hospitalização e foi reavaliada em consulta um mês depois, com resolução completa do caso. Não apresentou recorrência da úlcera até aos três meses seguintes. Conclusão: a úlcera de Lipschütz é uma doença subdiagnosticada, muito provavelmente porque os médicos não estão familiarizados com ela e por ser um diagnóstico de exclusão. A infeção pelo vírus Epstein-Barr é uma das etiologias propostas.

ABSTRACT

Diversas fontes de dados têm identificado a labioplastia ou ninfoplastia, dedicada à redução dos pequenos lábios, como uma intervenção estética muito procurada por mulheres e adolescentes brasileiras, sendo que o Brasil é o país que mais faz este tipo de cirurgia. Este ensaio discute como se promove e quem produz a divulgação desta imagem de aumento da demanda e do número de cirurgias realizadas. Está embasado em análise de artigos em periódicos científicos, material de imprensa e outros documentos; e ancorado no cruzamento entre os estudos sociais da ciência e da tecnologia, os estudos de gênero e a antropologia do corpo e da saúde. Por meio deste recorte, analisa como o campo da cirurgia plástica, ou mais precisamente seus representantes mais atuantes na mídia e na produção bibliográfica, pode atuar na criação de determinadas demandas de intervenção, por meio da promoção de normas classificatórias, tecnologias biomédicas e divulgação de resultados com ênfase nos benefícios projetados. Conclui que, apesar da falta de consenso sobre o que seria a hipertrofia dos pequenos lábios, a cirurgia tem sido muito indicada e praticada e contribui para a (re)produção de padrões estritos associados ao gênero feminino e a uma certa noção de "normalidade" corporal. Poucas referências mencionam que a divulgação das técnicas operatórias e mesmo deste tipo de cirurgia no consultório médico seria um fator relevante para entender o fenômeno. Sugere que o papel de médicos/as na promoção desta possibilidade de intervenção estética cirúrgica que, como qualquer prática deste tipo, não é isenta de riscos e efeitos inesperados, precisaria ser alvo de um debate mais aprofundado.


Various data sources have identified labiaplasty or nymphoplasty, aimed at the reduction of the labia minora, as an aesthetic intervention in high demand by Brazilian women and adolescents, with Brazil as the world's leading country in the performance of this surgery. This essay discusses how and by whom the dissemination of this image is promoted and produced, with the increase in demand and in the number of surgeries performed. The study is based on an analysis of scientific journals, press stories, and other documents. The approach employs a cross-analysis of social studies of science and technology, gender studies, and anthropology of the body and health. The article analyzes how the field of plastic surgery, or more precisely its more active representatives in the media and in research output, can act to create certain demands for surgical intervention through the promotion of classificatory norms, biomedical technologies, and the publication of results with emphasis on the projected benefits. Despite the lack of consensus on the definition of hypertrophy of the labia minora, the surgery has been widely indicated and practiced and contributes to the (re)production of narrow standards associated with female gender and a certain notion of the "normal" body. Few references mention that publicizing the surgical technique and even this type of surgery in the physician's office may be a relevant factor for understanding the phenomenon. The article suggests that the physician's role in the promotion of this possibility for aesthetic surgical intervention, like any such practice, is not free of risks and unexpected effects and should be the target of a more in-depth debate.


Diversas fuentes de datos han identificado la labioplastia o ninfoplastia, dedicada a la reducción de los labios menores, como una intervención estética muy solicitada por mujeres y adolescentes brasileñas, siendo Brasil el país donde más se realiza este tipo de cirugía. Este ensayo discute como se promueve y quién dirige la divulgación de esta imagen de aumento de la demanda y número de cirugías realizadas. Está basado en el análisis de artículos en periódicos científicos, material de prensa y otros documentos; y anclado en el cruce entre los estudios sociales de ciencia y tecnología, estudios de género y antropología del cuerpo y de la salud. Mediante esta reducción, analiza como el campo de la cirugía plástica, o más precisamente sus representantes más activos en los medios y producción bibliográfica, pueden actuar en la creación de determinadas demandas de intervención, mediante la promoción de normas clasificatorias, tecnologías biomédicas y divulgación de resultados con énfasis en los beneficios proyectados. Concluye que, a pesar de la falta de consenso sobre lo que sería la hipertrofia de los labios menores, la cirugía ha sido muy recomendada y practicada, así como que contribuye a la (re)producción de patrones estrictos asociados con el género femenino y a una cierta noción de "normalidad" corporal. Pocas referencias mencionan que la divulgación de las técnicas operatorias, incluidas las de este tipo de cirugía, en el consultorio médico sería un factor relevante para entender el fenómeno. Sugiere que el papel de médicos/as en la promoción de esta posibilidad de intervención estética quirúrgica que, como cualquier práctica de este tipo, no está exenta de riesgos y efectos inesperados, necesitaría ser objeto de un debate más profundo.

Introducción: El fibroadenoma es una lesión bastante común en mama, pero su localización en la región vulvar es extremadamente rara, con alrededor de sesenta casos, descritos en la literatura. Se presenta como una masa unilateral, subcutánea y asintomática, de crecimiento insidioso. El tamaño de este tipo de lesión varía entre 1 y 12 cm en los casos reportados hasta la fecha. A pesar de ser infrecuentes, las tumoraciones a nivel vulvar continúan siendo importante motivo de consulta. La histogénesis de este tipo de lesiones ha sido tema de discusión en los últimos años, sobresalen dos teorías: la presencia de tejido mamario ectópico, y la degeneración tumoral de un grupo de glándulas similares a tejido mamario, descritas como componente habitual de la región. Objetivo: Aportar un diagnóstico diferencial polémico de tumores vulvares. Presentación del caso: Se presenta una paciente de 22 años con diagnóstico histopatológico de fibroadenoma de vulva. Se realiza exéresis de la lesión, la cual al examen macroscópico midió aproximadamente 15 cm en su eje longitudinal (la más grande reportada en la literatura). Conclusiones: A pesar de su baja frecuencia de presentación, el fibroadenoma de vulva es una entidad a tener en cuenta como diagnóstico diferencial de los tumores subcutáneos de localización vulvar(AU)


Introduction: Fibroadenoma is a fairly common lesion in the breast tissue, but its location in the vulvar region is extremely rare, with about 60 specific cases in the literature. It is presented as a unilateral, subcutaneous and asymptomatic mass, with insidious growth. The size of this type of lesion is between 1 and 12 cm in the cases reported to date. Despite being infrequent, vulvar tumors continue to be important reasons for medical consultation. The histogenesis of this type of lesions has been the subject of discussion in recent years; two theories stand out: the presence of ectopic breast tissue, and tumor degeneration of a group of glands similar to breast tissue described as a common component of the region. Objective: To providea polemic differencial diagnosis of vulvar tumors. Case presentation: We present a 22-year-old patient with histopathological diagnosis of vulvar fibroadenoma. Excision of the lesion was performed, which at the macroscopic examination measured approximately 15 cm in its longitudinal axis (the largest reported in the literature). Conclusions: Despite its low frequency of presentation, vulvar fibroadenoma needs to be considered as differencial diagnosis of vulvar tumors(AU)

OBJECTIVES: We aimed to evaluate the effectiveness of microablative fractional radiofrequency (MAFRF) in the non-hormonal treatment of genitourinary syndrome of menopause. METHODS: We examined the cases of 55 postmenopausal women before and after treatment with regard to their vaginal health index (VHI), vaginal microbiota, vaginal pH, and cell maturation. Three applications of MAFRF were performed in the vagina/vaginal introitus. During the treatment, six vaginal smears were obtained and stained with the Papanicolaou stain for determining the degree of cell maturation and with Gram stain for classification of vaginal flora, as per the criteria of Spiegel and Amsel. For vaginal pH determination, pH indicator strips were applied against the vaginal wall. Statistical analysis was performed using SPSS for Windows (version 17.0). Data were reported as mean±standard deviation. The differences were analyzed using the statistical method of generalized estimation equations with autoregressive correlation structure "1" and robust standard errors. RESULTS: The mean age was 59.8±4.2 years, and the mean time of menopause was 15.4±4.5 years. After treatment, there was an increase in the percentage of Lactobacillus spp. (p<0.001). Consequently, there was a progressive decrease in vaginal pH during the treatment (p<0.001). Regarding cell maturation, there was a decrease in the percentage of parabasal cells (p=0.001) and an increase in the rate of superficial cells (p<0.001). Additionally, there was an improvement in the VHI index. The mean VHI values before and after treatment were 13.2±5.6 and 22.5±3.7, respectively (p<0.001). CONCLUSION: MAFRF treatment is well tolerated and leads to improvement in the vaginal microenvironment.

Introdução: Endometriose é a presença de tecido endometrial funcionante (glândulas endometriais e estroma) em localização fora da cavidade endometrial. Implantes endometrióticos extrapélvicos têm sido ocasionalmente descritos na literatura. Métodos: Este é um relato de caso de uma paciente do serviço de cirurgia plástica do autor, operada em setembro de 2013 em um hospital de Brasília-DF. O trabalho seguiu os princípios de Helsinque e o termo de consentimento livre e esclarecido foi realizado. Resultados: O trabalho se trata de uma paciente de 30 anos que apresentava dismenorreia, dor e parestesia em região inguinal direita, nódulo endurecido em grande lábio direito com cicatrizes de duas ressecções de focos de endometriose. A equipe de ginecologia realizou ressecção endoscópica das lesões cavitárias, do conteúdo do canal inguinal, do nódulo no grande lábio direito, da cicatriz umbilical e parte da aponeurose. Após se instalarem os defeitos de parede abdominal e vulva, a cirurgia plástica realizou a reconstrução com retalho de abdome inferior randômico, baseado lateralmente, como um dos vértices de uma zetaplastia. Conclusão: O retalho descrito no trabalho é uma alternativa na reconstrução da região inguinal e vulva, em casos de grandes defeitos, obtendo um resultado satisfatório.


Introduction: Endometriosis is characterized by the presence of functional endometrial tissue (endometrial glands and stroma) outside the uterine cavity. However, only a few cases of extrapelvic endometriosis have been described in the literature. Methods: This study reports the case of a patient from the author's plastic surgery service who underwent surgery in September 2013 at a hospital in Brasília, Federal District, Brazil. The study was conducted according to the principles of the Declaration of Helsinki, and the patient signed an informed consent form. Results: A 30-year-old female patient presented with dysmenorrhea, pain, and paresthesia in the right inguinal region as well as a firm nodule in the labia majora with scars from two surgical resections of endometriosis. The gynecology team performed endoscopic resection of the uterine cavity lesions, inguinal canal content, nodules in the right labia majora, umbilical scar, and part of the aponeurosis. The abdominal wall and vulvar defects caused by the resection were repaired using a laterally based random lower abdominal flap as one of the vertices of a Z-plasty. Conclusion: The flap described in this study is an alternative and satisfactory method for repairing large defects in the inguinal and vulvar regions.

ABSTRACT

Abstract Tuberculosis is an infectious disease caused by Mycobacterium tuberculosis. According to data from the World Health Organization, this disease remains one of the leading causes of death worldwide. Although it most commonly affects the lungs, tuberculosis can compromise any organ. The present study reports a rare case of vulvar tuberculosis in a postmenopausal woman with a history of asymptomatic pulmonary and pleural tuberculosis, with no prior documented contact with the bacillus. Diagnosis was based on vulvar lesion biopsies, with histological findings suggestive of infection and isolation of M. tuberculosis by microbiological culture and polymerase chain reaction (PCR) essays. The lesions reverted to normal after tuberculostatic therapy.


Resumo A tuberculose é uma doença infeciosa causada pelo Mycobacterium tuberculosis. De acordo com dados da Organização Mundial de Saúde, esta doença mantém-se entre as principais causas demorte nomundo. Embora afetemais frequentemente os pulmões, a tuberculose pode comprometer qualquer órgão. O presente artigo relata um caso raro de tuberculose vulvar numa mulher na pós-menopausa, com antecedentes de tuberculose pleural e pulmonar assintomática, sem contato documentado com o bacilo. O diagnóstico foi feito com base na biópsia da lesão vulvar, com achados histológicos sugestivos da infeção e isolamento do M. tuberculosis pormeios de cultura e pela técnica da reação em cadeia da polimerase (PCR). Após terapêutica tuberculostática, as lesões reverteram.

En el año 2000, había más de 120 millones de personas infectadas por filarias en el mundo; unos 40 millones están desfiguradas e incapacitadas por la enfermedad. La infestación de los ganglios linfáticos por el parásito Wuchereira bancrofti, es la causa más frecuente del linfedema secundario. Se presenta un caso inusual de elefantiasis vulvar por filariasis. Se analizan las alternativas terapéuticas que incluyen el tratamiento farmacológico y quirúrgico, con el fin de considerar esta última como un tratamiento paliativo más que curativo tanto para el linfedema como para la cirugía reconstructiva(AU)

Introduction: Inflammatory linear verrucous epidermal nevus (ILVEN) is a variant of verrucous epidermal nevus. It has a psoriasiform or eczematous and itchy aspect, and has differential diagnosis compared to other more common dermatoses; thus, histological studies are often necessary. It mainly affects women of early age and must be differentiated from condyloma acuminatum. Interestingly, the lower left limb is often involved, but the genital region is rarely affected. Treatment is refractory and the best method is not yet established. Objective: We present a case of unusual vulvar involvement known as inflammatory linear verrucous epidermal nevus. Methods: This was a clinical case report of a child diagnosed with ILVEN in the vulvar region. Case report: An 11-year-old female presented to the gynecology department of the Universidade Federal de Alagoas complaining of pruritic lesions on the large left vulvar lip, perianal and anal regions, and vaginal introitus. The lesions were hypochromic, eroded, and covered by scabs along the Blaschko line with verrucous lesions in the abdomen and upper and lower limbs. These characteristics fit the clinical criteria of Altman and Mehregan, and the histological criteria of Dupre and Christol for diagnosis of ILVEN. The treatment was performed with Vitanol A® and Epidrat Ultra® with partial improvement of the lesions. We chose to excise the lesions to control the condition. Conclusion: These lesions are characterized by recurrent inflammatory phenomena including psoriasiform or eczematous aspects in the extremities with genital involvement being rare. Other common dermatoses are often confused with ILVEN and make anatomically pathological analysis extremely important for diagnosis. Despite details on several types of treatment for ILVEN, there are no studies on relative advantages because this lesion is very refractory to the treatment.


Introdução: Nevo epidérmico verrucoso inflamatório linear (NEVIL) é uma variante do nevo epidérmico verrucoso. Tem aspecto psoriasiforme ou eczematoso e pruriginoso, sendo o diagnóstico diferencial com outras dermatoses mais comuns e o estudo histológico é necessário para diferenciálas. Afeta, principalmente, mulheres em idade precoce e deve ser diferenciado do condiloma acuminado. Curiosamente, o membro inferior esquerdo está envolvido, sendo a região genital raramente afetada. O tratamento é refratário e ainda não está estabelecido qual o melhor método. Objetivo: Apresentar um caso de incomum acometimento vulvar conhecido como nevo epidérmico verrucoso inflamatório linear (NEVIL). Métodos: Realizada documentação de caso clínico de criança com diagnóstico de NEVIL em região vulvar. Relato de caso: Uma paciente do sexo feminino, 11 anos de idade, procurou o serviço de ginecologia da Universidade Federal de Alagoas queixando-se de lesões pruriginosas vegetantes em grande lábio vulvar esquerdo, regiões perianal e anal e introito vaginal. As lesões eram hipocrômicas, erosadas, recobertas por crostas ao longo da linha de Blaschko com lesões verrucosas no abdome e nos membros superiores e inferiores. Essas características se enquadram nos critérios clínicos de Altman e Mehregan e aos critérios histológicos de Dupre e Christol para diagnóstico de NEVIL. O tratamento foi realizado com Vitanol A® e complexo hidratante (Epidrat Ultra®) com melhora parcial das lesões. Optou-se pela exérese das lesões com controle do quadro. Conclusão: Essas lesões se caracterizam por fenômenos inflamatórios recorrentes, incluindo aspecto psoriasiforme ou eczematoso, normalmente em extremidades, sendo raro o acometimento genital. Outras variedades de dermatoses mais comuns são frequentemente confundidas com NEVIL, tornando a análise anatomopatológica de extrema importância para o diagnóstico. Apesar dos detalhes sobre diversos tipos de tratamento para NEVIL, não há estudos sobre vantagens relativas entre eles, uma vez que essa lesão é muito refratária ao tratamento.

Introdução: A cirurgia de adequação genital tem se mostrado uma opção segura e confiável, com redução drástica na disforia e melhora da qualidade de vida das pessoas transgênero. A técnica mais estudada e utilizada é a inversão peniana com suas modificações, com aparência estética e funcionalidade adequadas, porém sem padronização da técnica cirúrgica. Índices de até 38% de satisfação parcial e 15% de insatisfação podem levar até 66% dos casos a realizar procedimentos adicionais. O objetivo é sugerir refinamentos estéticos na aparência da vulva e comparar com algumas das técnicas descritas, buscando aumentar a satisfação estética e funcional pós-operatória. Métodos: Estudo retrospectivo com 7 pacientes submetidas à cirurgia de readequação sexual entre agosto de 2017 e fevereiro de 2018. O clitóris é feito com a glande em formato de tridente, utilizando a coroa para construir os corpos cavernosos do clitóris e aumentar a área de sensação erógena. Faixa de prepúcio é usada para aumentar a cobertura do clitóris e cobrir a face interna dos pequenos lábios, que são definidos com o uso de suturas. Resultados: Sensibilidade adequada e satisfação com o resultado e capacidade de orgasmo em todas as pacientes observadas. Não houve estenose, fístula ou necrose do clitóris com essa técnica. Somente 1 caso precisou de procedimento adicional para melhor definição estética. Conclusão: A técnica apresentada tem alta satisfação das pacientes e sensibilidade erógena, com algumas vantagens em relação a outras técnicas. Porém, estudos prospectivos com número maior de pacientes são necessários para definir a técnica cirúrgica mais efetiva.


Introduction: Sex reassignment surgery is a reliable and safe option, which has drastically reduced dysphoria and improved the quality of life of transgender individuals. The most studied and used technique is penile inversion with modifications, which results in appropriate esthetic appearance and functionality, but the surgical technique has not been standardized. Partial satisfaction rates up to 38% and dissatisfaction rates of 15% may cause up to 66% of cases to undergo additional procedures. The objective is to suggest esthetic refinements to the appearance of the vulva and compare some of the techniques described, seeking to increase the postoperative esthetic and functional satisfaction. Methods: A retrospective study with 7 patients undergoing sex reassignment surgery between August 2017 and February 2018 was conducted. The clitoris is constructed with the glans in the form of a trident, using the corona to build the corpus cavernosa of the clitoris and increase the area of erogenous sensation. A section of the prepuce is used to increase the coverage of the clitoris and cover the inner surface of the labia minora, which are defined with the use of sutures. Results: Adequate sensitivity and satisfaction with the result and capacity of orgasm in all patients were observed. There was no stenosis, fistula, or necrosis of the clitoris with this technique. Only 1 case needed an additional procedure for better esthetic definition. Conclusion: The technique presented leads to high patient satisfaction and erogenous sensitivity, with some advantages compared to other techniques. However, prospective studies with larger numbers of patients are needed to define a more effective surgical technique.

We are going to describe the female soboliphymid nematodes, which were recovered from the stomach of a Asian badger, Meles leucurus (Mammalia: Mustelidae), in Geochang-gun, Gyeongsangnam-do, Korea. In February 1998, we found 2 peculiar nematodes with a cup-like organ in the anterior end from the stomach of badger. Recovered worms were fixed with 10% formalin, cleared in glycerin-alcohol solution and observed under a light microscope with a micrometer. They were 34.46 (33.43–35.50) mm long by 2.13 mm at maximum width. Cephalic sucker cup-like, 3.34 (3.13–3.55) mm wide, 2.40 (2.25–2.55) mm long, with the oral aperture and meridionally striated on the buccal capsule. Oral aperture 2.38 mm in diameter. Circumoral membrane 0.41 (0.38–0.45) mm wide. Esophagus muscular, 4.81 (4.50–5.00) mm long by 0.80 (0.78–0.83) mm at maximum width. Vulva situated at 3.13 mm ventro-anterior level from the esophago-intestinal junction. Vagina anteriad, 3.38 mm long, making a canal from the uterus to the vulva opening. Uterus single, large. Tail 0.35 (0.33–0.38) mm long. Intrauterine eggs long elliptical, 0.058–0.065 (0.062) mm long and 0.030–0.033 (0.031) mm wide. Based on the some morphological characters and host-specificity, our specimens are nearly identical with S. baturini. Therefore, the present report describes S. baturini for the first time in Korea.
